Legal considerations

The cadet’s parent / legal guardian is responsible to fully complete this form.

For the purposes of this document, the term “parent” refers to any individual holding guardianship over the cadet.

If the cadet has reached the legal age of majority or obtained legal emancipation (QC) or can demonstrate independent living to the satisfaction of the Corp / Sqn supervisor, they may complete this form on their own.

Age majority

AB, MB, ON, PE, QC & SK - 18

BC, NB, NL, NT, NS, NU, & YT - 19

Until the form is completed and processed, the cadet will be prevented from participating in ALL cadet training activities for safety reasons.

Membership application

When completing this form during the cadet’s initial membership application process, for each question answered "Yes" in the Basic Health Questionnaire section of the Online Registration Tool parents / guardians are asked to provide as many details as possible (date of onset, hospital visits, medications required, related surgeries, etc.)

Annual validation

When completing this form for annual validation purposes, parents / guardians are asked to answer every question and provide details even if they have done so before.

Changes in health status

When completing this form (due to changes in their child's health or medication), parents/guardians must answer all questions and provide details, even if they have done so before. Please also clearly indicate what is "NEW", "AMENDED" or "CANCELLED". If it is necessary to limit physical activity, a note from the family physician describing these limitations should be submitted with DND 2570 (Detailed Health Questionnaire). A medical note may be requested depending on the situation.

Medical condition disclosure

Cadet Medical Personnel will not divulge any medical information provided in this DHQ to non-medical individuals, including the CO of their Corp/Sqn, unless authorized by law.

If your child suffers from a health condition that you feel should be disclosed to the Corp/Sqn in order to ensure your cadet’s health and safety, we encourage you to communicate that information to the Corp/Sqn Commanding Officer and staff directly.

Privacy statement: The personal information you provide to DND/CAF is governed in accordance with the Privacy Act.

We only collect the personal information we need for the purpose of delivering, administrating and coordinating the medical care of persons attending Cadet Canadian Organizations (CCO) activities, which is authorized in accordance with Section 46 National Defence Act. Personal information may be shared with other medical professionals to ensure continuity of care.

In limited and specific situations, your personal information may be disclosed without your consent in accordance with subsection 8(2) of the Privacy Act. The refusal or failure to provide the requested personal information may prevent the cadet from participating within a cadet training activity / CTC and may result in degraded quality and/or inadequate medical care, jeopardizing the safety of the participant. In addition to protecting your personal information, the Privacy Act gives you the right to request access to and correction of your personal information.
