The personal information you provide to Health Canada will be used by the Natural and Non-prescription Health Products Directorate (NNHPD) under the Natural Health Products Regulations and handled in accordance with the Privacy Act.

Why are we collecting your personal information?  We are collecting your personal information to ensure that only designated parties are able to provide information on behalf of a company.

What are your rights?  You have the right to access and request a correction and/or notation to your personal information. You also have a right to complain to the Privacy Commissioner of Canada if you feel your personal information has been handled improperly. Natural Health Products Ingredients Database Request Form

Ingredients and/or associated information must be present in the Natural Health Products Ingredients Database (NHPID) to be selected in the Natural Health Product Licence Application Form (web PLA form) or the Natural Health Product Licence Amendment and Notification Form (ANF) prior to submitting an application in accordance with the Natural Health Products Management of Applications Policy.

This form is to be used to request modification(s) to the NHPID. All required fields must be filled out, accompanied by supporting evidence, to enable the form to be sent to the Natural and Non-prescription Health Products Directorate (NNHPD) for review.

Priority type

The requests for modifications to the NHPID are typically processed by date of receipt. However, depending on, e.g., the complexity and/or volume of requests, modifications required to be made to the NHPID for the submission of an application may be prioritized.
