Canadian Dental Care Plan online application feedback questionnaire

The information you provide is collected under the authority of the Department of Employment and Social Development Act and the Department of Health Act for the purpose of assessing and improving the service delivery of the CDCP application process.

Employment and Social Development Canada (ESDC) does not intend to collect any personal information from you through this questionnaire. ESDC will dispose of any personal information you might provide, as soon as practicable.
