“Our Community Flag Bearers” contest - Participation Form

Get involved today to celebrate the Canadian flag and showcase inspiring citizens! 

To participate, you must nominate someone you know who embodies the values symbolized by the Canadian flag (respect, inclusion, solidarity, etc.) and who deserves the title of Community Flag Bearer. 

You’ll need to explain in a few sentences why this person is a role model in their community. 

The person you nominate will be contacted by Canadian Heritage if their story is selected for public sharing and will be asked to give explicit consent prior to publication.

Privacy Notice 

The collection of personal information is authorized by paragraph 4(2)(h) of the Department of Canadian Heritage Act and is required to carry out the national flag day contest. Collection and use of this personal information are in accordance with the Privacy Act. This information will be used to contact the winners of the contest. The personal information collected is described in Personal Information Bank PSU 938 – Outreach Activities and will be retained for 10 years. Your submission may not be considered if you do not provide the required information. Under the Privacy Act you have the right of access to, and correction of, your personal information.
