The personal information you provide to Health Canada will be used by the Natural Health Products Program under the Natural Health Product Regulations (NHPR) and handled in accordance with the Privacy Act.

Why are we collecting your personal information? We are collecting your personal information for workload management and ensuring Canadians have access to products that are safe, effective, and of high quality. Your personal information will be used to validate responses and to ensure that only designated parties are able to provide product information on behalf of a company.

What are your rights? You have the right to access and request a correction and/or notation to your personal information. You also have a right to complain to the Privacy Commissioner of Canada if you feel your personal information has been handled improperly. This form allows applicants to identify product applications that will be manufactured in Canada within six months of receiving a product authorization.  The data collected from the use of this form will assist the Natural and Non-prescription Health Products Directorate (NNHPD) to focus its resources and review efforts.

Applicants must have a submission number prior to completing this form.  For more information on the NNHPD’s approach to workload management please review Bulletin No. 1 on our website.

To attest that your product(s) will be manufactured in Canada within six months of authorization, please provide us with the following information.
