Attestation Form on the Condition and Operation of the Vessel
For vessels participating in the pilot project to extend periodic inspection intervals as per the Tier I Policy – Pilot Project on the Inspection Periodicity for Small Certificated Domestic Vessels, this form must be completed and submitted to the local Transport Canada Centre for review 10 business days before the expiry of the vessel’s inspection certificate.

The information provided on this form will be used internally by Transport Canada to confirm if the vessel is eligible for an extension of its safety inspection certificate under the pilot project.
