Canadian Armed Forces Delayed Pension Class Action Claim Form

**Deadlines**: All claims  must be received by the Claims Administrator no later than 23:59 EST on Monday, 30 March 2026.

**Important:**It is the claimant's responsibility to ensure any changes to contact or mailing information after their claim has been submitted is reported to dpsp.claims-reclamations.dpps@forces.gc.ca.

PRIVACY STATEMENT

Personal information is required to assess your eligibility as a claimant for the Canadian Armed Forces Delayed Pension Class Action lawsuit. Your personal information is collected under the authority of section 4 of the National Defence Act and as a result of the Federal Court-approved Final Settlement Agreement (FSA) to administer and process your claim. Failure to provide the requested information will preclude the Department of National Defence and Canadian Armed Forces (DND/CAF) from processing your claim. A Payment Agent acting on behalf of Canada may be appointed to issue payments; in such circumstances, your personal information will be provided by DND/CAF to the court-appointed Payment Agent, solely for the purpose of issuing payments under the court-approved FSA. In all circumstances, your personal information will only be disclosed in accordance with the Privacy Act subsection 8(2).

The personal information you provide to DND/CAF is governed in accordance with the Privacy Act. This collection of personal information is described in Info Source Info Source - Canada.ca; refer to the personal information bank DND PPU 873. In addition to protecting your personal information, the Privacy Act gives you the right to request access to and correction of your personal information. For more information about these rights, please contact the DND/CAF Access to Information and Privacy Coordinator. You also have the right to file a complaint with the Privacy Commissioner of Canada regarding the handling of your personal information.
