Biosecurity Incident Notification Report

As per A 11.3 of the Biosecurity Addendum to the Canadian Biosafety Standard, Third Edition, following a biosecurity incident at a Containment Level 4 facility, a notification report is to be submitted to the Public Health Agency of Canada (PHAC) within two business days.

As per 4.8.11 in the Canadian Biosafety Standard, Third Edition, following a biosecurity incident at any containment level, PHAC is to be informed without delay via the submission of a notification report.

The PHAC will be collecting your information via the GC Forms tool. GC Forms is a product of the Canadian Digital Service (CDS), part of Employment and Social Development Canada. The CDS website has its own privacy practices.
