DND/CAF Fraud Risk Management Program Questionnaire
INSTRUCTIONS
You will be asked to reflect on your knowledge and experience related to fraud as well as fraud risk management, identification and reporting.
Context: Fraud is any intentional act of deception for unjust or illegal gain. Fraud may be committed by internal (e.g., Defence Team members) or external (e.g., third parties). The Fraud Risk Management Program (FRMP) led by Assistant Deputy Minister, Review Services aims to mitigate fraud risks in the Department of National Defence (DND)/Canadian Armed Forces (CAF).
Purpose: This questionnaire is designed to gather anonymous feedback from members of the Defence Team (DT). It helps us understand how aware people are of the organization’s efforts to prevent and detect fraud in order to refocus program efforts and resources as gaps are identified. The results will be used to track progress and measure the impact of the Fraud Risk Management Program (FRMP). The questionnaire is open to all Defence Team members.
Participation: The questionnaire will take approximately 10-15 minutes to complete. Your participation is confidential, anonymous and voluntary. You have the option not to answer any question that you are not comfortable with or that you feel may compromise your identity. This questionnaire will be open indefinitely and each Defence Team member may submit one response. Your input is crucial to the Fraud Risk Management Program as it helps us shape our program, making it responsive and tailored while identifying necessary course-corrections.
