The information you provide through this form is collected under the authority of the Canadian Institutes of Health Research (CIHR) Act, the Natural Sciences and Engineering Research Council (NSERC) Act, and/or the Social Sciences and Humanities Research Council (SSHRC) Act for the purposes of institutional eligibility review. Your information will be administered (collected, used, disclosed, and disposed) in accordance with the Privacy Act and other applicable laws, Treasury Board Secretariat policies, directives and guidelines, and the agencies’ retention and disposition schedules.
Providing your information is mandatory to be considered for institutional eligibility at CIHR, NSERC, and/or SSHRC. Refusal to provide your contact information will result in your organization’s institutional eligibility application being incomplete.
If your organization chooses to undergo a joint institutional eligibility review, the information you provide will be shared between CIHR, NSERC and/or SSHRC. This information will otherwise be kept confidential and will not be used or disclosed for any purpose outside of those indicated in this notice without your consent or as may be required by law and in compliance with the Access to Information and Privacy Acts.
Under the Privacy Act, you have the right to the protection of, access to, and correction of your personal information, which is described in CIHR’s Info Source Chapter – Bank number: CIHR PPU 005 and CIHR PPU 025, in NSERC’s Info Source Chapter – Bank number: NSERC PPU 060, and in SSHRC’s Info Source Chapter – Bank number: SSHRC PPU 055 or SSHRC PPU 016.
