This online form is for Canada Summer Jobs (CSJ) participants to report concerns about their job. It should only be used to share issues related to your safety, work environment, or rights at work. For general questions about your job or help accessing CSJ forms or links, please speak with your employer.

After you submit your concern, we will contact you to explain the next steps and, if necessary, may ask you for more details. You may request that your identity not be shared with your employer. If you do so, we will take steps to protect your privacy and will not disclose information that could identify you. Please note that we will use your personal information internally to review and address your concern and may contact you for additional information or follow-up.

Why your information is collected

The personal information you provide is collected under the authority of the Department of Employment and Social Development Act to review, assess, and respond to your concerns related to your Canada Summer Jobs employment experience.

Participation in submitting this form is voluntary. If you choose not to provide personal information we may not be able to follow up or take action on your concern.

When your information is disclosed

Your personal information may be shared within Employment and Social Development Canada and Service Canada on a need-to-know basis to address your concerns, including for program oversight, compliance and follow-up activities. If you choose to remain anonymous, we will not share information that could identify you with your employer.

The information may also be used and/or disclosed for policy analysis, research and/or evaluation purposes. However, this additional use and/or disclosure will never result in an administrative decision being made about you.

Your rights under the Privacy Act

You have the right to the protection of, access to, and correction of your personal information under the Privacy Act. Instructions for obtaining this information is outlined in the following government publication entitled Info Source.

You also have the right to request access to your personal information and to request corrections, should you believe the information you provided contains errors or omissions. To request a correction to your personal information, please contact the appropriate ATIP Coordinator.
