Surtax remission order: Short supply consultation

This form is intended to identify companies that manufacture the products listed in our consultation notice—those for which tariff remission has been requested on the basis of short supply—with the goal of helping Canadian importers find domestic alternatives.

Note: Please submit this form once for each business for which you are declaring products. If you are here on behalf of multiple businesses, you may restart the form after each submission to complete it for the next business.

Information received throughout this submission process is subject to the Access to Information Act and the Privacy Act and is collected under the authority of the Financial Administration Act. Participation in the consultation is voluntary and will have no impacts on services you receive from the Government of Canada. Personal information provided on this form is collected to enable individuals to participate in national and/or international outreach activities, as well as connecting businesses who are seeking remission due to insufficient supply with Canadian manufacturers who self-identify through this consultation.
