You do not need to have an existing appeal with the Employment Insurance (EI) Board of Appeal to use this form; it is available for everyone. Whether you are seeking information about the appeal process or need assistance as a prospective appellant, this form provides a streamlined way to make your request.

For information about an existing appeal with the EI Board of Appeal, you should contact the Registry Officer assigned to the appeal directly. Their contact information is included in the correspondence you received. However, if you do not have their contact information, you may use this form to request that the Registry Officer contact you directly, whether you are the appellant or a party to the appeal.

As an appellant or a party to the appeal, you may also use this form to provide your email address so that a secure electronic communication channel can be established with the EI Board of Appeal. This secure channel will enable you to send and receive documents pertaining to the appeal.

This form may be used to:
