Completion of this form is voluntary, however refusal to provide the requested personal information may result in the form being considered incomplete or ineligible for processing. Accommodation requests remain confidential and are not disclosed to other parties. If you mention accommodation needs in other parts of your appeal (e.g., in writing or during the hearing), the other party may see it as part of the evidence record. Your personal information is protected under the Privacy Act and accessible under the Access to Information Act. It will only be used and disclosed as described in the Personal Information Bank titled the “Employment Insurance Claim Files” (ESDC/PPU151).
